Care Navigation
Getting You to the Right Clinician, First Time
Our tried and tested model ensures you are directed to the most appropriate healthcare professional for your needs — which may not always be the GP.
This helps you get the right care quickly and efficiently.
Our Reception Team: Here to Help You
Our reception team is here to support and guide you to the most appropriate service. They may ask about the reason for your appointment so they can direct you to the right healthcare professional first time around.
This helps ensure you get the best care as quickly as possible.
Why We Operate Care Navigation
-
It helps provide faster access to general practice services.
-
It ensures patients see the most appropriate clinician or service to meet their individual needs — which may not always be a GP.
-
By working this way, it frees up GP time to focus on patients with complex or serious health conditions that require their expertise.
-
Most importantly, it means you are seen by the person or service best placed to manage your clinical problem.
Care Navigation is designed to improve your experience and ensure you receive the right care, at the right time.

Who Should I See?
We aim to reserve GP appointments for patients with serious health issues or conditions that only a GP can manage.
Before booking a GP appointment, please consider whether another clinician might be able to help, as many other healthcare professionals deal with a wide range of medical problems.
Our team is here to support you and will help decide the right person for you to see.
Pharmacy
Your local pharmacist is a highly trained professional who can provide confidential health advice for a wide range of common illnesses.
They can also help you decide if you need to see a clinician at the practice. Through the Pharmacy First scheme, you can get advice and treatment without needing an appointment.
NHS 111
Visit NHS 111 online at Get help for your symptoms - NHS 111.
NHS 111 is a free, non-emergency medical helpline. You should use this service if you urgently need medical help or advice, but it is not a life-threatening situation.
Call 111 if:
-
You need medical help but it is not a 999 emergency
-
You think you need to go to A&E or another NHS urgent care service
-
You do not know who to call
-
You need health information or reassurance about what to do next
A&E
A&E is for Life-Threatening Emergencies Only
Before attending A&E, please ask yourself: “Is this a real emergency?”
If it is not life-threatening, please consider using other local health services first to get the right care quickly and avoid unnecessary pressure on emergency departments.
